for patients ages 2 +.
How to Become a New Patient:
• Is a referral required? Yes, a referral is required by your primary care provider to be seen for non-urgent matters. If seen in the emergency room, you may be referred directly from the ER physicians
• Complete the New Patient Application and bring it to your appointment to expedite the check-in process and keep our appointments as timely as possible
• Please arrive 30 minutes prior to your first visit to ensure that all necessary paperwork has been completed.
